Book an Appointment

Before Booking an Appointment, please take the time to look at my new patient packet, which includes information on fees, confidentiality and consents. Please have the forms completed prior to your first appointment.  You can access the forms, download them, enter your information and securely file transfer them through the HIPAA Compliant Telehealth Video Conferencing application that we will be using for your appointment.

Your appointments will be videoconferenced over the Doxy.me platform.  It requires no download on your part or registration of an account.  No personal information is collected.  You will need a desktop or mobile device that has an updated version of Chrome, Firefox or Safari and must allow access to camera and microphone. 

New Patient Forms

You can use the online calendar to schedule your appointment. For an initial visit expect the process to take up to 50 minutes. Follow-Up appointments will be 25 minutes.
